A B-Form is the official birth registration document for Pakistani children under 18 — issued by NADRA using the parents' CNICs and a Union Council birth certificate, it is required for school enrolment, government scheme registration, and BISP child stipends.

4 Steps to Get a B-Form for Your Child

Gather father and mother CNICs, birth certificate, and Nikah Nama; apply online at nadra.gov.pk or visit a NADRA Facilitation Centre; B-Form issued same day or within 5 working days for Rs. 50–100.

Step 1

Understand the B-Form and When It's Needed

The B-Form (officially the Child Registration Certificate or CRC) is NADRA's identity document for children under 18. It links the child to their parents in NADRA's family tree database and is required for school enrolment, travel on a parent's passport, BISP Taleemi Wazaif registration, and applying for a CNIC at age 18.

B-Forms should be obtained shortly after birth — ideally within the first year. Late registration (after several years) is possible but requires more documentation. The B-Form number is a unique identifier for the child until they turn 18 and convert to a CNIC.

Step 2

Gather Documents for B-Form Application

Required documents:

  • Father's original CNIC
  • Mother's original CNIC (if available)
  • Hospital birth certificate or Union Council birth registration
  • Nikah Nama (marriage certificate)
  • Child's passport-size photograph

For children born outside a hospital (home birth), a Union Council birth registration certificate serves as the primary birth document. If no birth record exists, a sworn affidavit from the parents and a family witness can be used — NADRA's officer guides the alternative documentation process.

Step 3

Apply at NADRA or Online

Online: Visit nadra.gov.pk → Online Services → B-Form Application. Fill in the child's details, parents' CNICs, and birth information. Select your nearest NADRA office for document verification. You still need to visit to submit original documents and take the child's biometric photo.

In person: Visit any NADRA Facilitation Centre with the documents listed above. The process is straightforward — counter staff verify the parents' CNICs, enter the child's details, capture the child's photo, and issue the B-Form. For newborns, NADRA has a hospital outreach programme at major hospitals where B-Forms can be registered within days of birth.

Step 4

Receive and Safeguard the B-Form

B-Forms are typically issued the same day or within 3–5 working days. The fee is nominal (Rs. 50–100). Keep the B-Form in a safe place alongside the parents' CNICs — it will be needed at every school enrolment, medical registration, and government scheme application until the child turns 18.

Take a clear photo of the B-Form and store it digitally — losing the physical B-Form is common and a duplicate can be obtained at NADRA using the B-Form number, but having the digital copy means you always have the number available.

Common NADRA Application Problems

Hospital didn't issue a birth certificate

Get a birth registration from your Union Council — they can issue one based on the parents' declaration. Bring parents' CNICs and Nikah Nama to the Union Council.

Father is a Pakistani national but mother is foreign — any different process?

The child inherits Pakistani nationality through the father. The process is similar, but foreign mother's passport may be used in place of a CNIC. NADRA handles dual-nationality cases — call 051-111-786-100 for guidance.

Frequently Asked Questions

At age 18, the holder applies for a CNIC using their B-Form as the primary identity document. The B-Form details (name, parents, date of birth) carry over to the CNIC automatically since they're already in NADRA's database.

Children under 18 travelling on a parent's passport need their name endorsed on that passport — the B-Form confirms parentage for this purpose. Children travelling with their own passport need a separate child's passport; the B-Form isn't a travel document itself.
